The PA Child Protective Services Law changed recently. The change requires all coaches and volunteers involved with youth organizations to complete a State Police Criminal Record Check and a PA Child Abuse History Clearance. For those individuals who have not resided in PA continuously for the previous 10 years they must also complete an FBI Criminal Background Check.

If you HAVE resided in PA continuously for the previous 10 years, you must complete this affidavit.
If you have NOT resided in PA continuously for the previous 10 years, complete the FBI Criminal History Report Online Pay Portal

If you plan to volunteer as a coach or assistant coach, please complete these background checks and send scanned copies of your completed forms to [email protected].
